RESUMO

The incidence of postoperative nausea and vomiting (PONV) remains high, and improving the accuracy of PONV prediction remains challenging. The primary aim of this study is to examine the impact of anxiety scores evaluated using the Hospital Anxiety and Depression Scale (HADS) on the PONV prediction model. We hypothesized that anxiety and depression, quantified using the HADS, could improve the accuracy of the PONV predictive model. This pilot study evaluated 100 patients. The HADS was conducted by a self-evaluation method before thoracoscopic surgery for lung tumors, and the anesthesia method was standardized. The criterion was whether the nurse in charge of the patient who complained of PONV assessed that drug administration was necessary. As the main analysis, the odds ratio of the HADS score for predicting PONV was evaluated using multivariable logistic regression models. Further, the receiver operating characteristic (ROC) curves of the model with the HADS score added to the variables of without-anxiety predictors and the model with the variables of without-anxiety predictors only were compared. The anxiety score was significantly higher in the PONV group than in the no PONV group (P = 0.021). For predictive accuracy, the model that included age, sex, smoking history, history of PONV, and anxiety score had a higher area under the ROC curve than did the model excluding the anxiety score (P = 0.021). In conclusion, the findings indicate that the HADS is worth investigating as a predictor of PONV.

PURPOSE: To report adult patients with an orbital trapdoor fracture with extraocular muscle entrapment. METHODS: We retrospectively reviewed 566 adult patients (>18 years) with a pure orbital fracture who were referred to us from January 2016 to May 2023. The following data were collected: age, sex, affected side, causes of injury, concomitant ocular injury and nasal bone fracture, presence or absence of oculocardiac reflex and infraorbital nerve hypesthesia, period from injury to surgery, follow-up period, and pre- and postoperative limitation of extraocular muscle motility and fields of a binocular single vision. RESULTS: We found 5 patients (0.9%) with an orbital trapdoor fracture with extraocular muscle entrapment (age range, 19-47 years; all males; 2 right and 3 left). Causes of injury included performing a bench press, fall, assault, boxing, and bicycle accident. Entrapment of the inferior and medial recti muscles was seen in 2 and 3 patients, respectively. None of the patients had any sign of oculocardiac reflex. After surgical reduction, the field of binocular single vision became normal in 3 patients and was incompletely recovered in 2 patients, in whom consultation with us was delayed. CONCLUSION: Adults with extraocular muscle entrapment may not present with an oculocardiac reflex. Urgent release of an entrapped muscle is, however, still recommended to avoid permanent limitation of extraocular muscle motility.

Thromboelastography is a quantitative test widely used to measure the efficiency of blood clotting. However, awaiting the results of maximum amplitude (MA) is necessary for determining the need for platelet- and fibrinogen-containing products. A more rapid prediction of MA could facilitate faster preparation and administration of blood transfusion products, thereby resulting in coagulation improvement. In this retrospective study, we hypothesized that early amplitude at 10 min (A10) could be a predictor of MA. Therefore, we investigated whether MA can be rapidly inferred from thromboelastographic 6 s (TEG6s) measurements and evaluated its correlation with A10. We extracted TEG6s measurements obtained in operating rooms and intensive care units of our hospital between January 2018 and December 2022. The correlation of MA with display items of TEG6s results, including reaction time, kinetics, α angle, activated clotting time, and A10, was evaluated. The relationship between citrated rapid TEG (CRT)-A10 and CRT-MA, as well as between citrated functional fibrinogen (CFF)-A10 and CFF-MA, were evaluated if A10 and MA showed a good correlation. The results showed good correlations between CRT-A10 and CRT-MA, as well as between CFF-A10 and CFF-MA. Therefore, evaluating A10 using TEG6s could predict MA.

An eight-year-old boy presented with a one-month history of left eyelid swelling. The patient was diagnosed with periorbital cellulitis at another clinic and was treated with oral antibiotics. However, the swelling did not subside. On initial consultation, the patient had left upper eyelid swelling with erythema. His extraocular muscle motility was normal, and the results of blood tests were unremarkable. A computed tomographic scan demonstrated a mass in the superior orbit with destructive changes in the left frontal bone. Histopathological examinations revealed a dense infiltrate of histiocytic cells. Immunohistochemical staining of the tumors for CD1a and Langerin was positive. A pathological diagnosis of Langerhans cell histiocytosis was made. Since orbital Langerhans cell histiocytosis has a high risk for central nervous system involvement, chemotherapy was the treatment of choice for any residual lesion to prevent sequelae to the central nervous system. At the six-month follow-up, the lesion did not recur, and the patient did not experience any central nervous system sequela.

Subconjunctival herniated orbital fat (SHOF) usually occurs in the superotemporal quadrant, and SHOF located in other quadrants is extremely rare. This study aimed to compare the patient characteristics between patients with SHOF in the superotemporal quadrant and those with SHOF in other quadrants. This is a retrospective study of 45 patients with SHOF who were referred to our institution between January 2015 and January 2023 and 14 previously reported patients in published case reports of SHOF that were not located in the superotemporal quadrant. Data on patient age, sex, affected sides, and body mass index were collected from the medical records of our institution and from previous reports. Patient age, male-to-female ratio, unilateral-to-bilateral ratio, and body mass index were compared between patients with SHOF in the superotemporal quadrant and patients with SHOF in the other quadrants. Of the 45 patients, only 1 patient showed SHOF in a location other than the superotemporal quadrant (inferonasal quadrant). Based on a comparison of 44 patients with SHOF in the superotemporal quadrant and 15 patients with SHOF in the other quadrants, the latter entity predominantly occurred in young, non-obese females (P < .050). Unilateral cases were more common in SHOF in the other quadrants (P = .003). Patient characteristics were found to be different between cases of SHOF in the superotemporal quadrant and those with SHOF in the other quadrants.

Purpose: To examine changes in the position of the lower eyelid punctum and tear meniscus height (TMH) after correction of horizontal laxity of the lower eyelid in involutional lower eyelid entropion. Methods: This prospective, observational study included 42 sides of 36 patients with involutional entropion who underwent a lateral tarsal strip procedure or transcanthal canthopexy (+ lower eyelid retractor advancement). The horizontal distance from the medial margin of the lower lacrimal punctum to the medial canthus was measured using ImageJ software. TMH was measured using anterior segment optical coherence tomography. All measurements were performed preoperatively, at postoperative 3-month and at postoperative 6-month. Results: The lower lacrimal punctum significantly shifted laterally at 3-month follow-up and slightly returned toward its original position at 6-month follow-up (Friedman's test, P < 0.001). Although the differences did not reach statistical significance, TMH in the lower eyelid increased at 3-month follow-up and then slightly decreased at 6-month follow-up (Friedman's test, P = 0.076). Conclusions: The results of this study imply that lateral shift of the lower lacrimal punctum prevents effective drainage of tears accumulated in the lacrimal lake, resulting in increased TMH after correction of horizontal laxity of the lower eyelid in involutional entropion.

PURPOSE: The study aims to examine an incidence and risk factors of severe corneal involvement in patients with congenital lower eyelid epiblepharon. METHODS: This retrospective, observational study included 509 patients (933 eyes) with congenital lower eyelid epiblepharon. Data on age, sex, affected side, past history, past surgery, presence or absence of concomitant periocular/ocular diseases, body height and weight, astigmatic power, and corneal involvement were collected. Severe corneal involvement included corneal scarring, vascularization, and perforation. RESULTS: Severe corneal involvement was found in 30 patients (5.9%) (34 eyes, 3.6%). Corneal scarring and perforation with scarring were shown in 29 patients (33 eyes) and 1 patient (1 eye), respectively. Nine patients (12 eyes) demonstrated concomitant corneal vascularization with corneal scarring. Logistic regression analysis showed that body mass index had an odds ratio of 1.057 (P = 0.078). Age and sex did not affect occurrence of severe corneal involvement (P > 0.050). CONCLUSIONS: In this study, 5.9% of patients showed severe corneal involvement in congenital lower eyelid epiblepharon. Logistic regression analysis indicates that a high body mass index was a possible risk factor of severe corneal involvement caused by congenital lower eyelid epiblepharon.

PURPOSE: To examine the relationship between patterns of orbital floor fracture around the infraorbital groove and development of infraorbital nerve hypoesthesia. METHODS: This retrospective, observational study included 200 patients (200 sides) of pure orbital floor fracture with or without medial orbital wall fracture. Data on the presence or absence of infraorbital nerve hypoesthesia were collected from medical records. Based on coronal computed tomographic images, patients were classified into 3 groups: a fracture extending medially to (medial group), into (in-groove group), and laterally to the infraorbital groove (lateral group). RESULTS: Infraorbital nerve hypoesthesia was found in 72 patients (36.0%). A fracture extended into or laterally to the infraorbital groove in 86.2% of patients with infraorbital nerve hypoesthesia, while a fracture was limited to the portion medial to the infraorbital groove in 77.3% of patients without infraorbital nerve hypoesthesia (P < 0.001). A logistic regression analysis demonstrated that patients in the lateral and in-groove groups were highly associated with development of infraorbital nerve hypoesthesia, with an odds ratio of 134.788 in the lateral group (95% confidence interval, 30.496-595.735; P < 0.001) and that of 20.323 in the in-groove group (95% confidence interval, 6.942-59.499; P < 0.001) with the medial group as the reference. CONCLUSIONS: This study indicates that patients with orbital floor fracture extending into or laterally to the infraorbital groove have a high risk of infraorbital nerve hypoesthesia, compared to those with orbital floor fracture limited to the portion medial to the infraorbital groove.

BACKGROUND: Postoperative acute exacerbation of interstitial pneumonia has a high mortality rate; however, its treatment methods have not been standardized. CASE PRESENTATION: A 72-year-old man with rheumatoid arthritis developed acute respiratory failure about 3 weeks after lung cancer surgery. There were increased diffuse frosted shadows in both lung fields. His condition was diagnosed as an acute exacerbation of interstitial pneumonia associated with rheumatoid arthritis, and he was started on steroid pulse therapy; however, his respiratory condition deteriorated. He was urgently intubated and started on veno-venous extracorporeal membrane oxygenation. Further, intensive care, including blood purification therapy, was initiated. The blood purification therapy comprised a combination of hemodiafiltration and 6-h polymyxin B-immobilized fiber column direct hemoperfusion. The patient was weaned off veno-venous extracorporeal membrane oxygenation, extubated, and discharged from the intensive care unit on the ninth day. CONCLUSIONS: Blood purification therapy was effective for acute exacerbation of interstitial pneumonia.

A 39-year-old man presented with a five-day history of swelling of the right upper eyelid and ocular irritation in the right eye. On the first examination, the patient showed conjunctival injection, conjunctival chemosis, swollen upper eyelid, and palpable lacrimal gland with tenderness on the right side. Magnetic resonance images showed an inflamed right lacrimal gland. Blood test demonstrated negative results for immunoglobulin M of Epstein-Barr, mumps, herpes simplex, and herpes zoster viruses. We administered oral prednisolone (30 mg/day) based on a possible diagnosis of idiopathic dacryoadenitis. One week after steroid treatment, the periocular inflammation reduced to some extent although the inflammation substantially persisted. Four weeks after the steroid treatment, the patient informed us that he had met his friend 10 days before the onset, and that friend had conjunctival injection at that time which was subsequently diagnosed as an epidemic keratoconjunctivitis. The periocular inflammation subsided, but two corneal white spots were observed on slit-lamp examination. Although immunochromatographic test for adenovirus was negative, the blood test showed a positive result for immunoglobulin M of adenovirus serotype 3. In eight weeks of follow-up, the number of corneal opacities increased to five spots, but the acute dacryoadenitis did not recur.

A 48-year-old man visited the emergency department of our hospital with swelling of the left upper and lower eyelids from the day before. On the first examination, he had severe swelling of the left upper and lower eyelids, proptosis, and chemosis. Left intraocular pressure was 33 mmHg. Computed tomographic images showed an orbital abscess in the anterosuperolateral orbital space, maxillary and ethmoidal sinusitis, and dacryocystitis. The orbital abscess was not contiguous to maxillary and ethmoidal sinusitis and dacryocystitis. Ground-glass appearance was seen in the frontal, maxillary, and ethmoid bones, and most of the space of the frontal sinus was obliterated due to the expansion of the frontal bone. Emergent drainage of orbital abscess, dacryocystorhinostomy, and endoscopic sinus surgery were performed under general anesthesia. Intravenous tazobactam/piperacillin was administered. A culture test of the sinus pus and orbital abscess showed growth of Streptococcus intermedius (2+). At one month postoperatively, there was no recurrence of orbital abscess, paranasal sinusitis, and dacryocystitis.

BACKGROUND: Orbital metastasis from pancreatic tumors is extremely rare, and its clinical characteristics are still unclear. CASE PRESENTATION: Our case was a 73-year-old female who noticed diplopia on right gaze 3 months before referral to us. Imaging studies demonstrated a mass involving the lateral rectus muscle in the right orbit. The results of pathological examination of an excised specimen corresponded to poorly differentiated adenocarcinoma. Systemic work-up revealed pancreatic carcinoma with peritoneal metastasis. The patient underwent chemotherapy. We reviewed literature on similar cases and found 19 reported cases of pancreatic tumors metastasizing to the orbit. The results of our review indicate a tendency for formation of solitary mass without bony erosion, delayed detection of the primary pancreatic carcinoma, and poorer prognosis of such tumors, compared to metastatic orbital tumors from other lesions. CONCLUSIONS: We report a rare case of metastatic orbital tumor from an unknown primary pancreatic carcinoma. Clinical characteristics of cases with metastatic pancreatic tumors seem to be different from those with metastatic tumors from the other lesions. Pancreatic tumors are frequently asymptomatic in an early stage, leading to delayed detection of the primary pancreatic carcinoma and poorer prognosis.

RESUMO

An 80-year-old female complained of diplopia after undergoing cataract surgery. On the first examination, adduction and abduction were slightly restricted. Magnetic resonance imaging revealed a mass in the medial rectus muscle. The results of pathological examination of a specimen harvested from the mass corresponded to granular cell tumor. Although we could not completely excise the mass because of firm adhesion of the mass to the muscle, there was no recurrence without any suspicious metastatic lesion at 2.5 years of follow-up.

PURPOSE: To diagnose central serous chorioretinopathy (CSC) by deep learning (DL) analyses of en face images of the choroidal vasculature obtained by optical coherence tomography (OCT) and to analyze the regions of interest for the DL from heatmaps. METHODS: One-hundred eyes were studied; 53 eyes with CSC and 47 normal eyes. Volume scans of 12×12 mm square were obtained at the same time as the OCT angiographic (OCTA) scans (Plex Elite 9000 Swept-Source OCT®, Zeiss). High-quality en face images of the choroidal vasculature of the segmentation slab of one-half of the subfoveal choroidal thickness were created for the analyses. The 100 en face images were divided into 80 for training and 20 for validation. Thus, we divided it into five groups of 20 eyes each, trained the remaining 80 eyes in each group, and then calculated the correct answer rate for each group by validation with 20 eyes. The Neural Network Console (NNC) developed by Sony and the Keras-Tensorflow backend developed by Google were used as the software for the classification with 16 layers of convolutional neural networks. The active region of the heatmap based on the feature quantity extracted by DL was also evaluated as the percentages with gradient-weighted class activation mapping implemented in Keras. RESULTS: The mean accuracy rate of the validation was 95% for NNC and 88% for Keras. This difference was not significant (P >0.1). The mean active region in the heatmap image was 12.5% in CSC eyes which was significantly lower than the 79.8% in normal eyes (P<0.01). CONCLUSIONS: CSC can be automatically diagnosed by DL with high accuracy from en face images of the choroidal vasculature with different programs, convolutional layer structures, and small data sets. Heatmap analyses showed that the DL focused on the area occupied by the choroidal vessels and their uniformity. We conclude that DL can help in the diagnosis of CSC.

PURPOSE: To report the clinical characteristics of eyes with an unmeasurable small size of foveal avascular zone (FAZ) in the optical coherence tomography angiographic (OCTA) images. METHODS: Two-hundred sixty-seven eyes of 255 patients (mean age 60.4 years) without retinal and choroidal disorders to cause any type of visual impairment were examined by optical coherence tomography angiography (OCTA; RTVue XR Avanti, Optovue, Fremont, CA). Cross-sectional images at the fovea (DRI-OCT, Topcon, Japan) and fundus autofluorescence (CX-1 MYD/NM, Canon, Japan) were also recorded from all eyes. RESULTS: Four eyes (1.5%) of 3 patients (2 men, 1 woman; average age, 63.3 years) were found to have an unmeasurable small size of FAZ in the OCTA images. The best-corrected visual acuity was better than 20/20 in all eyes. Cross-sectional OCT images showed the presence of a foveal depression and the inner retinal layers in the foveal depression. These inner retinal layers were detected as a hyperreflective bands at the fovea. Fundus autofluorescence showed hypo-autofluorescence at the fovea as in normal eyes. CONCLUSIONS: An unmeasurable small size of FAZ without visual impairment was detected in 1.5% of 267 normal eyes. These eyes may be classified as low-grade foveal hypoplasia.
